Family Caregiver Support / Caregiver's Respite

The Haven Transitions program offers informaton, resources and referrals to individuals and their caregivers who are coping with a life-limiting or terminal illness. Every month Haven Transitions helps more than 250 people who can benefit from pre-hospice care.

These services are offered at no charge:

Supportive phone calls
Volunteer assistance for companionship and respite care
Children's anticipatory grief support group (call for locations)
Information about and referrals to community resources
Education to help with understanding care options and choices
Advance care planning: living wills and advance directives
